We offer customized Support Coordination to individuals with an allocated NDIS Plan.
The role of a Support Coordinator is to assist you to work through your NIDS plan to navigate and link you into appropriate services as outlined in your NDIS plan goals. This can include NDIS registered services as well as non-registered and community services.
My Way Support Coordination Services aims to improve your independence and wellbeing outcomes by supporting you to achieve your goals documented in your NDIS plan. This may include support with access to the community to an activity of choice, access to appropriate psychological support, travel support, support with activities of daily living and support with sourcing suitable housing and employment services.
The aim of the Support Coordinator is to assist and empower the participant to self-manage their plan in the future.
How is Support Coordination delivered?
Support Coordination can be delivered via a range of modes:
> Face to face home visits
> At an agreed community venue
> At the Support Coordinators place of work
> Telehealth meeting,
> Phone and text messages.
We endeavor to provide the initial meeting face to face where possible.
As Support Coordinators we need to ensure we use our allocated hours wisely as we often do not receive sufficient funding to provide regular home visits.
Support Coordination is a dynamic role which involves:
. care team meetings
. regular liaison with service providers, participants, their carers, families, and the NDIA.
. Support Coordinators are expected to write reports when requesting a plan variation/reassessment and or change of circumstances as well an 8 week and 9 month Support Coordinator Report.
What to expect at the initial meeting?
The initial meeting is an opportunity to meet with your NDIS Support
Coordinator face to face. You will have the opportunity to go through the goals
documented in the plan and discuss how they will be implemented as well as
identify order of priority. This is your opportunity to clarify anything that you
may not understand about your plan and or how the NDIS operate.
You will go through the My Way Support Coordination Service agreement to
ensure you have a clear understanding of the expectations of the Support Coordinator.
What does the Support Coordinator consider when sourcing providers and
equipment?
1. Is the service or equipment requested value for money?
2. Is the item or service requested reasonable and necessary?
3. Choice and control of the participant
4. Can mainstream services provide the same and or similar support?
5. Is the service specifically for people with a disability?
6. Is it a standard mainstream and or everyday item that the community is
expected to have access to? Ie washing machine.
7. Can the participant fund the service or item?
8. Will the service and or item improve the independence and quality of life
for the participant?
9. Is the request in line with the NDIS guidelines?
How is Support Coordination Funded?
Support Coordinators are allocated several hours ranging from 30 to 120 hours, depending on the identified goals of the participant. Support Coordinators charge in 15-minute increments according to the NDIS price guideline. For example, a phone call and email would equate to 15 minutes of service.
